How to use this form:
1. Enter the height and width in the boxes below (to the nearest mm, i.e. 42.6cm)
2. Use the Quantity box if you need identical frosted panels.
3. Choose a design from the Plain & Designs page, and enter its code in the Design Code box.
4. If you wish to compare two different designs for a panel, enter a 2nd code in the Alternative Design Code box.
5. Choose a Position if applicable and/or enter any special instructions in the Additional Notes box.
6. Enter your contact details at the bottom of this page and click Submit.
